A sign for the rehabilitation flats

We start discharge planning very early in your stay to enable us to identify how we can support you when you leave.

A ‘Discharge Planning Meeting’ (DPM) will be held with you and all relevant parties to summarise your progress during your rehabilitation at the NRC and to co-ordinate onward plans following discharge.

Following assessments, which may include a home visit, the team will refer you for appropriate equipment that is available to facilitate a safe and timely discharge. Referrals will be made to relevant agencies such a district nurses, care agencies and therapy follow-up where required, and any other agency that may assist enabling you to safely go home. We will also refer to other outpatient services if this is required.

Before you leave the NRC to go home, we may be able to offer you and your close family members who will be living with you the opportunity to stay in one of our two rehabilitation flats.

These allow patients and their families to practice living independently, whilst still having our staff on hand if needed.
